If you are diagnosed with a long term or chronic condition you will be invited to attend an annual review on the month of your birthday. You may also be required at other times of the year for interim reviews.

- Peripheral arterial disease
- Rheumatoid arthritis
- Secondary prevention of coronary heart disease

Long Term Conditions (LTCs) are health conditions that can impact on a person’s life and may require ongoing care and support. We organise appointments and provide support for people who have an LTC by putting in place an annual recall system.
You will be invited to book your LTC review in your month of birth. On receipt of your invitation you will be able to book the appointment by contacting us between 11am and 4pm Monday to Friday.
As part of your review you may be sent a SMS and asked to complete a remote consultation. All the responses you provide are automatically saved into your medical record and available for the nurse and/or doctor to review.
If you are invited for a face-to-face review you may be asked to have your bloods taken a week before your appointment date. It is very important that you have your bloods taken as the results will inform your discussions with your health professional together with any agreed action plans.
